1. How soon should I arrive for my intial visit/evaluation and what information will be needed?
You should arrive at least 15 minutes prior to your appoinment time in order to complete necessary paper work. If your physician has given you a referral slip (prescription) please bring it with you. We will also need to make copies of your drivers licence and insurance card. For worker's compensation claim, please bring employer information number.
2. How long will my physical therapy visits typically last?
Your first visit/initial evaluation is typically 60 to 90 minutes. Each follow up visit will vary depending upon the complexity of your problem and typically will last from 30 to 90 minutes.
3. What type of clothing should I wear?
Please wear loose fitting comfortable clothing. Gym attire such as sneakers, T-shirts or tank tops and shorts or sweatpants are appropriate.
4. What is your cancellation policy?
When possible we ask that you please give 24 hours notice prior to canceling your appointment. By doing so, hopefully we can help someone else in your place.
5. Do I need to have a doctors order to begin physical therapy?
Yes. The Physical Therapy Practice Act in the state of Indiana requires a prescription for physical therapy services from either a medical doctor, dentist, podiatrist, chiropractor. A prescription can also be accepted from a nurse practitioner providing that proper documention has been provided authorizing the nurse practitioner to do so on behalf of the physician.
6.Will my insurance pay for physical therapy services?
We are credentialed with most major insurance companies. Please refer to our inusrance page for a complete listing. While it is the responsibility for the patient (insurance policy holder) to know their insurance benifits, our office staff will make a courtesy call to your insurance carrier to verify your physical therapy benifits.
7. Will I see the same physical therapist every visit?
Yes! We pledge to make every effort to ensure that you see the same physical therapist every visit. Exceptions may occur during sickness, vacation and continuing education coverage.
